Overview of Supplemental Term Life Insurance Enrollment

USC offers Supplemental Term Life Insurance if you wish to purchase extra coverage in addition to your Basic Term Life Insurance. The after-tax cost of Supplemental Term Life Insurance is based on your age and the additional coverage amount you choose, which is a multiple of (from 1 to 5) of your base salary rounded up to the nearest $1000.

Instructions

You start the Supplemental Term Life Insurance Enrollment by selecting it from the Benefits Type Selection Page.

  1. On the Online Benefits Enrollment & Transaction Page, select "Regular Enrollment" for the Enrollment Type. Use "Regular Enrollment" even if it's during Benefits Open Enrollment.
  2. Select the type of transaction you want to make:
      "Start" a new enrollment,
      "Change" your current Supplemental Term Life Insurance plan, or
      "End" your Supplemental Term Life Insurance.
  3. Click on the "Continue" button.
    • If you are Starting or Changing your Supplemental Life Insurance plan, the Coverage Selection Page will load.
    • If you are Ending your Supplemental Life Insurance plan, the Summary Review Page will load. (Skip the next step.)
  4. The Coverage Selection Page appears only if you are starting or changing your plan.

    1. Select the supplemental amount of coverage that you want. Supplemental coverage selections are based on a multiple of (from 1 to 5 times) your current base salary rounded up to the nearest $1000.
      Please be aware that supplemental coverage amounts over $750,000 require approval through underwriting, and that supplemental coverage is significantly reduced for the insured who are age 65 and over.
    2. Click on the "Continue" button to go to the Summary Review Page.
  5. On the Summary Review page, your current Supplemental Term Life plan, if you have one, and your new plan are displayed. Review your plan and read the authorization message.

    Click on the "Submit to Update" button. By clicking on this button, you are acknowledging that you have read and agree with the Authorization statement.
  6. You will see the final Confirmation message page. Print out this page for your records. The system will also send you a confirmation email.

Enrollment & Transaction Selection Page

Upon selecting Supplemental Term Life Insurance Enrollment on eTrac, you need to select the type of enrollment and transaction you want to make. The options available to you depend on the time of year and whether or not you are currently enrolled in a plan.

Instructions

  1. Select an Enrollment Type:
    Select "Regular Enrollment", even if it's during Benefits Open Enrollment.
  2. Select a Transaction Type:
    "Start"
    Select "Start" if you do not have Supplemental Term Life Insurance and want to enroll in it.
    "Change"
    If you have Supplemental Term Life Insurance and want to change the plan, select "Change".
    "End"
    If you have Supplemental Term Life Insurance and want to end the plan, select "End".
  3. Click on the "Continue" button to continue.
    You will go to the Coverage Selection Page if you are starting or changing your plan.
    If you are ending your Supplemental Term Life Insurance, you will go to the Summary Review Page.
     

Employee Information Section

The top section of this page displays your basic Employee information and it is repeated on every page. If any of this information is incorrect, contact your Home Department Coordinator to update your data.

Employee ID  This is your 7-digit USC Employee ID # and you use it to access all your employee/payroll records.
Employee Name  This is the name as how it appears on your record in the system. If it needs to be corrected, contact your Home Department Coordinator.
Employee SSN  This is your Social Security #. For your protection, only the last 4 digits of your SSN are displayed.
USC ID  This is your 10-digit USC ID number.
Employee Street Address, City, State, Zip  This is your home mailing address. You can update this from the Personal Information Update Page on eTrac.
Home Department  This is the name of your home department.
Home Department Coordinator  This is the name of the person to contact regarding your personnel file.
Employee Status  This is your current employee status at USC.
Employee e-mail  This is your USC e-mail contact. Since everything you do on the eTrac system sends you a confirmation notice via e-mail, make sure it's correct.
Employee Pay Frequency  This is how often you are paid.

Enrollment & Transaction Selection

Enrollment Type  Select an Enrollment type:
Regular Enrollment
Select "Regular Enrollment". Use "Regular Enrollment" even during Benefits Open Enrollment.
Transaction Type  Select a Transaction Type:
Start
Select "Start" if you do not have Supplemental Term Life Insurance and want to enroll in it.
Change
Select "Change" if you want to change your current Supplemental Term Life Insurance plan.
End
Select "End" if you want to end your current Supplemental Term Life Insurance.

Action Buttons

The following buttons appear near the bottom of the page:

Continue  After selecting the enrollment and transaction types, click on this button to continue.
Back to Benefits Page  To cancel any changes, click this button to return to the Benefits Type Selection Page.
Exit Process  Click on this button to return to the main menu.

Coverage Selection Page

The Coverage Selection Page appears only if you are starting or changing your Supplemental Term Life Insurance plan. Select the amount of supplemental coverage that you want. The supplemental coverage amount is a multiple (from 1 to 5 times) of your current base salary rounded up to the nearest $1000. Note, however, that if your coverage amount exceeds $750,000, underwriting is required. Also, if you are aged 65-69, your coverage is 65% of the eligible amount; and if you are aged 70 and over, your coverage is 50% of the eligible amount.

Instructions

  1. Select a Coverage Multiplier of 1 to 5 times of your current base salary. Your supplemental coverage amount will be rounded up to the nearest $1000.
  2. Click on the "Continue" button and the Review Page will load.

Current Supplemental Term Life Insurance Plan

This section displays your current Supplemental Term Life Insurance plan if you have one.

Plan Description  This is your Supplemental Term Life Insurance plan which is based on your age.
Plan Start Date  This date is when your plan begins.
Plan End Date  This date is when your plan ends.
Employee Rate  The Employee Rate is the cost of your plan based on your age for each $1000 of coverage.
Biweekly/Monthly Deduction  The deduction amount is the plan cost that is deducted from your pay, either monthly or biweekly. The monthly employee premium is calculated by dividing your Coverage Amount by 1000 and multiplying by your Employee Rate. If you are on a Biweekly pay frequency, the monthly premium is divided between 2 biweekly pay checks in the month.
Multiplier  This number is the multiple of your base salary you selected for your coverage amount.
Coverage Amount  The coverage amount is your base salary multiplied by your selected multiplier and rounded up to the nearest $1000.

Coverage Selection

Select Coverage Multiplier  Select the number of times (1 to 5) to multiple your base salary. The total rounded up to the nearest $1000 will be your supplemental coverage amount.

Action Buttons

The following buttons may appear near the bottom of the page:

Continue  Click on this button to continue.
The Summary Review Page will load.
Back to Enrollment Page  Click on this button to cancel the enrollment and return to the Enrollment Selection page.
Back to Benefits Page  Click on this button to cancel changes and return to the Benefits Selection page.
Exit Process  Click on this button to cancel changes and return to the eTrac menu.

Summary Review Page

The confirmation page displays a summary of your changes and plan details. Please review the information. If everything looks in order, then click on the "Submit to Update" button located near the bottom of the web to submit your plan request. Your enrollment transaction is not complete until you press the Submit button on the Review Page. Your request will be forwarded to the Office of Benefits Administration for review. You should also receive an email confirmation regarding your submission, and Benefits Administration will contact you in the near future on the status of your request.

Instructions

  1. Review all the information, including the authorization message near the bottom of the page. When you click on the final "Submit" button, you are acknowledging that you have read over everything and you have authorized the deduction from your pay.
  2. Click on the "Submit to Update" button. You will see a final confirmation message page.

    If you do not want to submit the request, click on the "Back to Contribution Selection Page" button or the "Benefits Selection Page" to make further changes, or click on the "Exit Process" button to return to the eTrac menu.

Current Supplemental Term Life Insurance Plan

This section displays your current Supplemental Term Life Insurance plan if you have one.

Plan Description  This is your Supplemental Term Life Insurance plan which is based on your age.
Plan Start Date  This date is when your plan begins.
Plan End Date  This date is when your plan ends.

Read the message near the bottom of the page carefully. When you click on the final "Submit" button, you are acknowledging that you have read over everything and you have authorized the deduction from your pay.

Action Buttons

The following buttons appear near the bottom of the page:

Submit to Update  Click on this button to submit your enrollment. You have not submitted your enrollment transaction until after you click on this button. You will see the Confirmation page.
Back to Coverage Selection Page  Click on this button to cancel changes and return to the Coverage Selection page.
Back to Benefits Page  Click on this button to cancel changes and return to the Benefits Selection page.
Exit Process  Click on this button to cancel changes and return to the eTrac menu.

Confirmation Message Page

The message page confirms that you have successfully submitted your request. It may include additional messages regarding your enrollment. You are advised to print out the message page for your records.

Action Buttons

The following buttons appear near the bottom of the page:

Exit Process  Click on this button to go to the eTrac menu.
