The Employee Voluntary Deductions Inquiry Page displays your current and prior payroll deductions taken for voluntary plans.
The top section of this page displays your basic Employee information.
Current Voluntary Deductions Section
This section lists all voluntary plans in which you are currently an active participant and require payroll deductions.
Prior Voluntary Deductions Section
This section lists all voluntary plans in which you previously had payroll deductions, but are no longer currently an active participant.
Section Table Header Descriptions
The Voluntary Deductions are displayed in a table format with the following column headers.
|Description||This column lists the name and description of the voluntary plan.|
|Start Date||This column displays the date when you start active participation in the plan. For many plans, payroll deductions are taken the month prior to the actual start date.|
|End Date||This column displays the last day of your active participation in the plan, or the last date of the current rate or set-up. If your participation continues beyond that date, the plan administrator will adjust the end date accordingly. If no end date appears, the item will continue until you or the administrator takes action to make a change.|
|Deduction Amount||This column displays the regular deduction rate/amount of the voluntary plan.|
|Coverage Amount||If the voluntary plan provides a monetary coverage amount, this amount is displayed under this column.|
|Contact Info||This column displays whom to contact for more information about the plan/deduction.|
